We strongly believe that routine immunizations are in your child’s best interest. We will be happy to discuss any questions or concerns you may have. Recommended well child visits include:
- Newborn visit (2 days from hospital discharge)
- 1 month
- 2 months
- 3 months
- 4 months
- 5 months
- 6 months
- 9 months
- 12 months
- 15 months
- 18 months
- 2 years
- 2 ½ years
- 3 years and up, annual visit
We are happy to fill out forms for daycare, school, sports, etc. at the time of the well visit without charge. Otherwise, there is a $5 fee and a 2 (business) day turnaround time. We do not fax forms due to HIPPA regulations.
Please note that forms cannot be filled out and referrals cannot be made if your child has not received their routine well child visits in our office. For referrals, please give us at least 48-72 hours (excluding weekends) notice before your scheduled appointment with the specialist.
